Signing up for cheaper prices

*Children MUST be enrolled with a parent/caregiver
i.e. A parent/caregiver has to be enrolled with the practice for us to accept the child enrolment.
WHY SHOULD I ENROL?
Enrolling means you sign up to a practice. The government gives us funding, so we are able to make your consultation cheaper. It's free and quick. You really should do it.
WHO CAN ENROL?
New Zealand cititzens
Permanent Residents
Work Visa for 2 years or more
Australians who plan to live in New Zealand for 2 years or more
Diplomats
A few other conditions which are so rare - we have to look it up. If you are not in the top 5, you pretty much can't enrol. This includes people with student visas, work visas less than 2 years (yup - even if it's 1 year and 364 days it doesn't count).
OK I WANT TO ENROL. HOW DO I ENROL?
Option 1:
Use this link to fill out the form online. Make sure you have your Passport/Visa/Birth Certificate with you. You can take a picture of it as proof of eligibility.

Option 2:
Download the enrolment form with the button below, by clicking the "Download Enrolment Form".
Fill it in. Scan or take a photo of it and email it back to us at: reception.baysidemedical@gmail.com
Scan or take a photo of your passport with your citizenship, PR or 2 year work visa and email it to us.

Alternatively - come to the front desk and we'll sort it out with you.
